Home health care is a service, prescribed by a physician, to assist with rehabilitation after an illness, injury, hospital stay, or surgery. We also help manage patients with a chronic medical condition with the goal of preventing unnecessary hospitalization. Through one-hour home visits from specialized clinicians, we work with our clients until recovery goals are achieved. PrimRose home health care visits are generally completely covered through the Medicare home health benefit or by private insurance.
Customized Care Based On Your Needs
Whether you reside in Logan or Box Elder County, you can be confident you will receive first-class clinical care from PrimRose. Each care plan is coordinated in collaboration with your physician and expertly delivered by our highly-trained staff. A licensed PrimRose care coordinator will visit your home to discuss what care is necessary and covered by Medicare or private insurance. Then, the road to rehabilitation may begin. Your PrimRose personalized care team of specialized clinicians visits your home, usually up to one hour at a time, until your individual goals are met. Depending on your needs, the following professionals may be involved in the process:

• Occupational therapists (OTs), to improve the ability to perform everyday activities
